What is GBHA?

Greater Baltimore Health Alliance (GBHA) is a Care Transformation Organization that brings doctors and other healthcare providers together with your health insurer, including Medicare, to give you the best possible care. All providers in GBHA use the latest Electronic Health Records to more carefully track your care, and make sure your doctor has the most up-to- date information about your health.

Care Transformation Organization

GBMC is committed to advancing primary care so that you receive the best possible care. The State and Federal government have launched a program — the Maryland Primary Care Program (MDPCP) — to help more primary care physicians get closer to advanced primary care. The program offers primary care practices additional funding for care management and access to an interdisciplinary care management team—through an entity called a Care Transformation Organization (CTO). GBMC is a champion of advanced primary care, and through Greater Baltimore Health Alliance (GBHA), has been approved to serve as a CTO.

Care Transformation

Care Management

This partnership offers a customized care team, focused on helping patients meet their goals and supporting the primary care provider. The care team provides care transition services and follows evidence-based clinical care pathways to improve patient’s health and well-being.

In addition to the provider, the care team may include:
  • Nurse Care Managers – Provides education and resources to help manage chronic diseases.
  • Care Coordinators - Helps patients understand the healthcare system and coordinates medical and social services.
  • Behavioral Health Consultants - Proactively addresses a wide range of mental health and addiction disorders

Community Health Needs Assessment

Greater Baltimore Medical Center (GBMC) performs a comprehensive Community Health Needs Assessment (CHNA) every three years to evaluate the health needs of individuals living in Baltimore County, Maryland. The goal of the survey is to reinforce GBMC’s commitment to the health of residents and align its health prevention efforts with the community’s greatest needs.
