Care Tranformation Organization

GBMC is committed to advancing primary care so that you receive the best possible care. The State and Federal government have developed a new program—the Maryland Primary Care Program—to help more primary care physicians get closer to advanced primary care. The program offers primary care practices additional funding for care management and access to an interdisciplinary care management team—through an entity called a Care Transformation Organization. On July 31st, 2018, GBMC, through GBHA, was approved to serve as a Care Transformation Organization.
