What is GBHA?

Greater Baltimore Health Alliance (GBHA) is a Care Transformation Organization that brings doctors and other healthcare providers together with your health insurer, including Medicare, to give you the best possible care. All providers in GBHA use the latest Electronic Health Records to more carefully track your care, and make sure your doctor has the most up-to- date information about your health.

Care Transformation Organization

GBMC is committed to advancing primary care so that you receive the best possible care. The State and Federal government have launched a new program—the Maryland Primary Care Program—to help more primary care physicians get closer to advanced primary care. The program offers primary care practices additional funding for care management and access to an interdisciplinary care management team—through an entity called a Care Transformation Organization. GBMC is a champion of advanced primary care, and through GBHA, has been approved to serve as a Care Transformation Organization. As a Care Transformation Organization, we are committed to supporting practices that are eager to deliver high quality patient care.
